Council for Affordable Quality Healthcare (CAQH) ProView Credentialing Application

CAQH is an online database that collects information for the credentialing process.

Completing the CAQH online credentialing application is a vital step of the onboarding process. This application is time sensitive and must be completed at least 8 weeks prior to your start date. Please begin this process immediately.

Note: Delay in completing your CAQH application may result in a delay of your start date. You do not need to have your licenses to start working on your CAQH application.

Before you start the CAQH application you will need your CAQH Provider ID Number.

Below are a set of instructions for completing this online application:

Getting Started – New Users:
  1. Go online to https://proview.caqh.org/pr
  2. Click "Register."
  3. Enter your CAQH Provider ID Number.
  4. Enter your authentication data (e.g., SSN, DOB, etc.).
  5. Create Username and Password.
  6. Choose and answer three security questions.
  7. Acknowledge the Terms of Service.
  8. Complete the application and review data.
  9. Authorize Kaiser Foundation Mid-Atlantic to access your information.
  10. Verify your data entry - attest.
  11. Submit supporting documents.
Getting Started – Current Users:
  1. Go online to https://proview.caqh.org/pr
  2. Enter your current CAQH Username and Password.
  3. Follow the prompts to create a new Username and Password.
  4. Choose and answer three security questions.
  5. Acknowledge the Terms of Service.
  6. Click "Update Account."
  7. Confirm the CAQH Provider ID we have on file for you is correct.
  8. Proceed to the CAQH ProView home page.
  9. Complete the application and review data.
  10. Verify your data entry - attest.
  11. Submit supporting documents.
